SUPPORTED LIVING

Our Supported Living programs offer assistance, care, and community integration and let individuals take control of their lives.

From licensed community homes to apartments with in-home care, we have various residential settings for individuals to call home.

Anne Grady Services’ Supported Living program operates 15 homes in Lucas County. These high-quality residences are in neighborhoods throughout the community.

Supported Living allows us to provide choices, encourage independence, and offer appropriate assistance to individuals with intellectual disabilities. Anne Grady’s Supported Living services range from basic in-home aid to 24-hour supervision.

Funded by Medicaid Individual Options (IO) and other waivers, the program provides wide-ranging assistance. Clients and staff work together to develop Individual Service Plans (ISP) that fit each person’s desires and needs.
