Anne Grady Services has eight Intermediate Care Facilities (ICFs). The main location, Anne Grady Center, is a 53-bed ICF located on Eber Road in Holland, Ohio. The facility—positioned in a peaceful wooded setting—boasts a therapy pool, an activities center, multiple distinct homes, and a highly qualified staff.

Our other ICF homes are on Everwood Road, Eileen Road, Hill Avenue, South Avenue, Dorr Street, and Zone Avenue in Toledo, as well as Eber Road in Holland. The seven homes combine to serve individuals with intellectual disabilities with the highest quality care possible.
